SPEECH AND LANGUAGE PATHOLOGIST: JOB DESCRIPTION
JOB REQUIREMENTS
● Master’s Degree in Speech-Language Pathology from an accredited program.
● Certificate of Clinical Competency or eligibility for Clinical Fellowship Year
● Current state licensure or license eligible
● Assumes responsibility for ongoing continuing education and professional development
PHYSICAL REQUIREMENTS
Physical demand for this job is classified as “light work” which includes
● Exerting up to 20 pounds of force occasionally
● Exerting up to 10 pounds of force frequently
● Walking or standing to a significant degree
● Sitting while constantly pushing or pulling materials whose weight is negligible
● Stair climbing occasionally
● Overhead reach infrequently
● Average workday is up to 8 hours

JOB RELATIONSHIPS
Responsible to and reports to the Director of Therapy/Rehab
JOB RESPONSIBILITIES
1. Provides and directs speech therapy services to residents, including assessment, treatment, program
planning and implementation, related documentation, and communication. Functions under
physician’s orders.
2. Provides speech therapy services to residents and initiates referrals when appropriate and in
accordance with Northwoods Rehab Policies and Procedures.
3. Conducts screenings of residents at regular intervals to determine need for intervention and/or
treatment.

4. Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts
specialized evaluations as indicated.
5. Interprets and communicates evaluation findings to residents, family and other team members.
6. Establishes individual resident schedules in conjunction with other team members.
7. Develops treatment plans by identifying a target list, including short and long-term goals and
methods to achieve identified goals.
8. Coordinates treatment plan with resident, family and other team members.
9. Implements and conducts treatment as outlined in treatment plan.
10. Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in
accordance with physician orders.
11. Develops appropriate home or programming within the community to maintain and enhance the
performance of the resident in their own environment.
12. Documents results of resident’s assessment, treatment, follow-up and termination of services.
13. Adapts treatment to meet the individual needs of the resident.
14. Recommends and obtains appropriate equipment to improve resident’s communication. Trains
resident, family and appropriate staff in the use of augmentative communication devices.
15. Instructs resident, family and staff in appropriate speech-language procedures or home program to
allow carryover of therapeutic remediation and goals in daily activities.
16. Participates in discharge planning.
17. Supervises Rehabilitation Techs/Aides, students and volunteers.
18. Reviews the quality and appropriateness of the total services delivered and of individual speech
therapy programs for effectiveness and efficiency, using predetermined criteria.
19. Participates in resident, family and staff education.
20. Confers formally and informally with other team members in coordinating the total rehabilitation
program of the resident.
21. Adheres to Northwoods Rehab procedures regarding documentation of speech therapy services.
22. Attends in-service meetings and community meetings as required.
23. Provides in-service education to the members of the resident’s care team and education to the
community.
24. Provides treatment in accordance with established standards of practice and department
procedures.
25. Maintains a safe and clean work area and adheres to community safety standards, as well as
complying with all Infection Control, Universal Precautions and OSHA standards for the healthcare
professional.
26. Consistently demonstrates sound judgment in the evaluation, planning implementation and follow up of resident speech therapy programs.
27. Maintains positive relationships and rapport with coworkers, supervisors, residents, family members
and community personnel and administration.
28. Assists with and participates in Quality Assurance Program as required.
29. Adheres to established confidentiality standards of the resident, as well as the organization.
30. Reports to work on time and coordinates schedule for efficient patient treatments.
31. Projects a positive and professional image at all times.
32. Maintains passwords for the software required for position and keeps all passwords secure.
