Patient Care Coordinator – Abortion Navigation, Bilingual Portuguese Speaking

For nearly 50 years, we have addressed the complex needs of people who are disadvantaged by poverty and discrimination and have fallen through the cracks of mainstream systems of care. Program: Health Imperatives Sexual & Reproductive Health Services

Fulltime, Monday through Friday including some evenings

$32/HR

Medical, dental, holiday, paid earned leave, 403b, etc.

Patient Care Coordinator will work within the Sexual and Reproductive Health Services team and in partnership with patients to help address barriers to care and achieve better health outcomes by:

  • Serve as the primary point of contact for patients seeking abortion services
  • Conduct patient pre-visit intakes, review consent processes, appointment expectations, and logistical needs
  • Assist patients with insurance navigation, prior authorizations, Abortion Fund navigation, and billing
  • Triage patient concerns pre and post medication and procedural abortion services
  • Track patient referrals, support patient navigation to outside medical care, provide follow-up post referral, and obtain medical records for clinician review
  • Serve as primary medical assistant during abortion clinics
  • Develop and maintain an up to date referral list of surgical abortion, medical, and social service providers
  • Complete accurate and timely documentation in the EMR, logs, and report system
  • Acting as a liaison within HI programs and with community providers
  • Providing patient-centered sexual and reproductive health education and counseling on topics including pregnancy options, abortion services, contraception, sexually transmitted infections, HIV, Hepatitis, PrEP, nPEP, gender affirming hormone therapy, overdose education and narcan distribution
  • Other duties as assigned

Bilingual Portuguese speaking; associates degree and at least 2 years of experience providing patient health education, care coordination, and/or or case management; strong client-centered skills; demonstrated ability to communicate unbiased reproductive health knowledge to adolescent and adult patients in an effective and culturally competent, non-judgmental manner; experience working with diverse populations; excellent written and oral communication skills in English; work independently as well as collaboratively within a team environment; computer literacy essential.

Bachelor's degree in health or human services or minimum of five years experience; case management or health navigation experience particularly regarding sexual and reproductive health and abortion related support services; demonstrated experience providing sexual and reproductive health education.
