• Conduct thorough assessments of patients' psychosocial needs and develop tailored treatment plans that align with their individual circumstances.
• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy, to help patients cope with their conditions.
• Maintain accurate, up-to-date, and confidential patient records using our secure digital documentation system.
• Work in close collaboration with a multidisciplinary treatment team, including medical professionals, nurses, and other social workers, to ensure holistic patient care.
• Regularly review the latest research and best practices in clinical social work to continually enhance your therapeutic techniques.

Your primary focus will be on patients facing end-of-life challenges, including those dealing with terminal illnesses, chronic pain, anxiety, and depression. You will support not only the patients but also their families, offering guidance and resources during an emotionally taxing journey. Your role will be pivotal in helping them navigate feelings of loss, grief, and fear, thus contributing directly to improved patient and family satisfaction.

• A Bachelor’s degree in Social Work (BSW) from an accredited institution.
• Active Licensed Clinical Social Worker (LCSW) license in the state of Georgia.
• Demonstrated proficiency in clinical assessments, interventions, and patient advocacy.
• Exceptional verbal and written communication skills to effectively convey empathy and understanding.
• A strong ability to work independently while contributing as a collaborative team player.

• Previous experience in a hospice or palliative care setting is highly desirable.
• Familiarity with treating patients dealing with trauma, anxiety, depression, and significant life changes.
• Proficiency in using telehealth platforms to connect with clients remotely.
