Key Responsibilities

• Engage effectively with interdisciplinary teams consisting of nurses, physicians, and other health professionals to create and implement comprehensive care plans tailored to meet the unique needs of each client.
• Conduct thorough case reviews, utilizing evidence-based methodologies to craft actionable treatment plans that focus on symptom management and emotional support.
• Provide education and resources to clients and their families, empowering them with the knowledge needed to navigate the complexities of hospice care.
• Monitor responses to treatment and adjust care approaches as required, ensuring each client's evolving needs are met with diligence and compassion.
• Stay updated on the latest advancements in mental health practices, incorporating innovative strategies into your work to enhance patient engagement and outcomes.
• Actively participate in team meetings, sharing insights and experiences to contribute to a collaborative workplace environment.

Patient Population / Client Focus

In this role, you will specifically serve clients receiving hospice care, often dealing with terminal diagnoses. This demographic presents unique challenges, requiring sensitivity and specialized skills to support emotional and psychological well-being. You will work closely with both clients and their families, helping them cope with grief and loss while fostering an environment of understanding and support.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Active New Jersey LCSW license is essential for this role.
• Demonstrated proficiency in clinical evaluations and the design of personalized treatment plans.
• Excellent interpersonal skills combined with the ability to collaborate across a multidisciplinary team are crucial for success.

Preferred Experience

• While a minimum of two years of relevant experience is required, preference may be given to candidates with direct experience in hospice or palliative care settings.
• Experience utilizing electronic health record (EHR) systems for tracking patient progress and documenting care plans is highly advantageous.

Compensation and Benefits
