Work Environment

This position offers the exciting flexibility of remote work while ensuring you remain a vital part of our collaborative healthcare team. Regular business hours will encompass weekdays, with occasional evening and weekend shifts as needed according to patient caseloads. Key Responsibilities
  • Conduct in-depth psychosocial assessments through interviews, observations, and medical record reviews, evaluating emotional, social, and environmental factors that impact patient recovery.
  • Provide empathetic counseling and emotional support for patients and their families, assisting them in navigating adjustment issues related to their rehabilitation journey.
  • Collaborate closely with a multidisciplinary rehabilitation team, including physicians, therapists, and other social workers, to develop and implement individualized treatment plans that align with each patient’s psychosocial needs.
  • Guide patients in accessing valuable community resources, including local support groups, vocational rehabilitation services, and housing assistance programs to further enhance their recovery.
  • Educate patients and their families about available community resources and post-rehabilitation services to ensure a smooth transition back into their everyday lives.
  • Participate in comprehensive discharge planning with the rehab team, facilitating transitions to home or other care facilities to promote continuity of care.
Patient Population / Client Focus

The role focuses on a diverse patient population, including individuals recovering from substance use disorders, traumatic brain injuries, strokes, and other conditions requiring intensive rehabilitation. As a Licensed Clinical Social Worker, you will play a vital role in supporting the entire rehabilitation process, enhancing emotional resilience and nurturing independence in our patients.

Qualifications
  • A Master's degree in Social Work from an accredited institution is mandatory.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice.

  • A solid understanding of psychological adjustment, grief, and coping mechanisms in the context of rehabilitation is essential.
Preferred Experience
