• Conduct in-depth psychosocial assessments to evaluate clients’ mental health needs, strengths, and areas for growth.
• Implement a variety of evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patient requirements.
• Collaborate closely with a multidisciplinary team that includes psychiatrists, nurses, and case managers to coordinate comprehensive care for clients.
• Provide education and clarification to patients and their families regarding diagnoses, treatment options, and medication management.
• Facilitate individual, group, and family therapy sessions using trauma-informed and culturally competent principles.
• Develop and maintain treatment plans in conjunction with the treatment team, integrating patient goals and preferences into the care process.
• Assist clients in navigating mental health resources and advocate for their rights and needs within the healthcare system.
• Lead psychoeducation groups, therapeutic workshops, and discharge planning sessions to ensure a continuum of care and support.

• A Master’s degree in Social Work from an accredited institution.
• A valid and active state license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated proficiency in mental health disorders and treatment options, along with crisis intervention strategies.
• Strong interpersonal skills and a collaborative mindset, conducive to working within a team-oriented environment.

• Experience with telehealth services and familiarity with electronic health record (EHR) systems is highly desirable.
• Prior experience in community health settings or working with underserved populations is a plus.
• Knowledge of and experience in integrating holistic approaches and community resources into treatment plans.
