Hospital Admissions Liaison

Fully Remote | Full-Time | Friday, Saturday & Sunday
Three 12-Hour Shifts | $23–$25/hour

We are hiring a fully remote Hospital Admissions Liaison to support weekend admissions for our portfolio of long-term care and skilled nursing facilities. You will work alongside our other Hospital Liaisons reviewing and coordinating referrals from hospitals throughout the St. Louis region, Springfield area, and surrounding communities.

Qualifications
  • Long-term care or skilled nursing admissions experience required

  • Must be able to confidently read and interpret hospital referrals, medical records, and clinical documentation

  • Nursing or clinical background is recommended, but not required

  • Strong computer skills and ability to navigate multiple referral systems

  • Organized, detail-oriented, and able to work independently

  • Must reside in IL or MO

Responsibilities
  • Review and screen incoming hospital referrals

  • Identify key clinical and admissions information

  • Create a clear, concise synopsis of each referral

  • Route referrals to the appropriate facilities and admissions teams

  • Communicate with facility teams to help coordinate weekend admissions

Schedule & Pay
  • Friday, Saturday & Sunday

  • Three 12-hour shifts / 36 hours per week

  • Full-Time

  • 100% Remote

  • $23–$25 per hour
