Location:Remote, Location anywhere in the U.S.

Type:W2, Full-Time, Benefits Eligible

Schedule:

  • Full Time: 40 hours per week
  • Work Schedule:Position is primarily focused on daytime rounding, with at leastthree daytime rounding shifts weekly. Remaining hours may include evenings, weekends, and flexible coverage shifts as needed.

About the Role

We are seeking a compassionate, mission-drivenFamily MedicineNurse Practitionerto join ourtelehealth team. This role is ideal for a dedicated provider who wants to make a meaningful impact servingunderserved populationswhile working remotely in a flexible schedule.

As a Telehealth Nurse Practitioner, you will be responsible for conducting virtual patient visits, assessing symptoms, and creating thoughtful, effective care plans. Strong clinical judgment, excellent documentation, and clear communication are essential to success in this role.

Responsibilities

  • Provide telehealth care to patients of multiple settings through theNeverAloneprogram;
  • FNP/CNP serving patients of all ages in diverse settings with strong emphasis serving underserved populations in SNF/LTC
  • Perform thorough assessments and determine the best plan of care
  • Ensure accurate, timely, and complete documentation of all encounters
  • Collaborate with care teams to support continuity of care
  • Deliver compassionate, patient-centered service to underserved communities

Qualifications

  • Active Nurse Practitioner licenseintwoof the following states:Maryland, West Virginia, Virginia, Ohio, or Indiana.
  • Company will reimburse expenses for any additional state licenses needed
  • DEA license required
  • 3-5+ years of experienceas a Nurse Practitioner
  • Family NP certification
  • Experience inacute, post-acute, nursing home, palliative care,&/orED settingsstrongly preferred
  • Strong ability to interpret patient symptoms and apply best practices in treatment
  • Mission-driven, empathetic, and committed to serving vulnerable populations
  • Excellent communication and documentation skills
