Great Lake Counseling & Coaching Center is actively seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. • Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs, ensuring a thorough understanding of each patient’s unique situation.
• Provide individual and group counseling sessions targeting issues such as adjustment disorders, grief, and coping mechanisms, tailored to the diverse backgrounds of our patient population.
• Collaborate actively with medical teams to create holistic discharge plans, connecting patients to community resources and services post-discharge to ensure successful outcomes.
• Offer emotional support and education to patients and their families, guiding them through various treatment options and available resources tailored to their specific needs.
• Participate in hospital committees and meetings, contributing your expertise on social work-related matters to enhance the organization's service delivery.
• Maintain accurate documentation consistent with hospital policies, ensuring compliance with legal and ethical standards of practice.

You will work with a diverse patient population, ranging from children to seniors, addressing mental health challenges and psychosocial issues that often accompany physical health concerns. The focus will be on empowering individuals and families by providing continuous support and guidance throughout their healthcare process, fostering a sense of agency and recovery.

• A Master’s Degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to conduct psychosocial assessments effectively.
• Exceptional communication and interpersonal skills, necessary for building rapport with patients and collaboration with medical teams.

While not mandatory, two years of experience in a healthcare or hospital setting is highly preferred, particularly within community health environments. Familiarity with medical terminology and crisis intervention techniques will be beneficial in effectively supporting patient needs.
