Voice Counseling and Life Skills Consultation Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in supporting patients and families within a Hospice Center setting. Your day-to-day responsibilities will include:

• Providing direct clinical services to clients within the hospice care framework, including conducting assessments, individual therapy, group therapy, and family sessions.
• Creating and implementing comprehensive integrated care plans that are tailored to the unique needs of clients and ensure effective coordination of services.
• Conducting brief, evidence-based screenings and arranging for in-depth assessments when necessary, ensuring all actions adhere to established ethical guidelines and best practices.
• Participating as an active team member, contributing to shared goals, improving service delivery, and enhancing patient outcomes through collaboration and shared insights.
• Assisting with crisis interventions to address acute psychiatric concerns while continuously monitoring client progress against treatment objectives.
• Maintaining accurate and timely documentation of client interactions, treatment plans, and progress reports, ensuring compliance with all regulatory and funding requirements.
• Being part of on-call rotations to provide crisis support, ensuring clients have access to treatment during critical times.

In this role, you will primarily work with clients who are facing terminal illnesses, providing emotional support and mental health treatment to both patients and their families. Understanding the unique challenges of hospice care, you will employ a trauma-informed approach that respects the diverse backgrounds and experiences of the individuals we serve, ensuring they receive holistic support tailored to their cultural and emotional needs.

To be successful in this position, you must fulfill the following qualifications:

• A valid Licensed Clinical Social Worker (LCSW) credential.
• A Bachelor's Degree in Social Work or a related field; a Master’s Degree is preferred.
• Demonstrated commitment to serving diverse populations and an understanding of the challenges faced by under-served communities.
• Expert knowledge of assessment, diagnosis, treatment methodologies, and crisis management specific to mental health.
• Excellent verbal and written communication skills to foster effective collaboration with clients and interdisciplinary team members.
• Strong organizational skills to manage a diverse caseload efficiently and maintain required documentation.

While a minimum of two years of clinical experience is required, candidates with direct experience in hospice or palliative care settings will be prioritized. Knowledge of therapeutic interventions, including CBT, DBT, and trauma-informed practices, will significantly enhance your effectiveness in this role.
