• Conduct thorough mental health assessments that take into account patients' co-occurring medical conditions, ensuring a holistic understanding of their individual challenges.
• Develop and implement integrated treatment plans that address the interplay between psychiatric and medical needs, ensuring an effective treatment pathway tailored to each patient.
• Facilitate individual and group therapy sessions using evidence-based methodologies, adapting each session to align with the unique circumstances and goals of the patients.
• Collaborate closely with medical providers to coordinate treatment efforts, ensure continuity of care, and maximize positive outcomes for our patients.
• Provide immediate crisis intervention and support for patients experiencing mental health emergencies, utilizing your expertise to navigate high-pressure situations.
• Educate patients on the relationship between mental health and physical wellbeing, empowering them with strategies and resources to actively participate in their own care.
• Maintain accurate clinical documentation that adheres to ethical standards and legal requirements, ensuring confidentiality and security of patient information.
• Participate in regular team meetings and interdisciplinary case discussions to contribute to care planning and share insights on best practices.
• Stay abreast of recent research and developments in integrated care approaches, incorporating new evidence into your practice to enhance patient care.

In this role, you will primarily work with a diverse clientele comprising individuals with complex mental health challenges, many of whom also face co-occurring medical issues. Your ability to understand and address the unique needs of each patient will be crucial in helping them navigate their mental health journeys and achieve improved outcomes.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Valid licensure as an LCSW, LPCC, or LMFT in the state of California.
• Demonstrated experience in supporting the mental health needs of individuals with co-occurring medical conditions.
• Exceptional interpersonal and communication skills, with a strong emphasis on patient engagement and rapport-building.
• Proficiency with electronic health record systems and telehealth platforms, ensuring efficient documentation and patient interaction.
• Ability to maintain professional boundaries, adhere to ethical guidelines, and ensure patient confidentiality at all times.

• Experience working in a community health or outpatient setting with a focus on integrated care solutions.
• Familiarity with evidence-based treatment modalities and therapeutic approaches applicable to diverse patient populations.
• Track record of successful collaboration within multidisciplinary teams, contributing effectively to patient care and treatment planning.

The position offers a competitive salary range of USD 115,000 to 130,000 annually, along with a generous sign-on bonus of $3,000. Our comprehensive benefits package includes:
