• Conduct individual counseling sessions with clients through secure video platforms, focusing on their mental health needs and overall emotional well-being.
• Facilitate discussions that allow clients to express their emotions and experiences, fostering insight into their personal situations and relationships.
• Advocate for clients by helping them articulate their goals and objectives, keeping their best interests at the forefront of treatment plans.
• Assess clients' mental health status and create customized treatment plans drawing from evidence-based practices and solution-focused methodologies.
• Collaborate with other healthcare professionals within the hospice care setting to ensure comprehensive and integrated care for clients.
• Maintain accurate records of client progress and prepare notes in compliance with regulatory standards, while minimizing the burden of documentation on yourself.

The clients you will work with primarily include stable adults experiencing low-acuity mental health concerns, such as anxiety, depression, and grief related to end-of-life issues. All clients are pre-screened to assure they are suitable for treatment and do not include severe mental health cases. This focused approach allows you to employ your expertise effectively while making significant contributions to a vulnerable population at a pivotal time in their journey.

• A Master's degree in Social Work (MSW) from an accredited institution.
• A current and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW), allowing for independent and unsupervised practice.
• Strong critical thinking and decision-making abilities to formulate effective treatment plans tailored to individual client needs.
• Exceptional interpersonal skills, demonstrating empathy and understanding in all client interactions.
• Proficiency in using technology and online platforms to conduct virtual counseling sessions.

• Prior experience in hospice care or a related field is highly valued.
