You will collaborate closely with fellow clinicians, support staff, and management teams through teleconferencing and electronic communications, ensuring seamless patient care and case management. As a Licensed Clinical Social Worker at Support Wellness Healthcare Services, your responsibilities will include:

• Conducting comprehensive clinical evaluations to assess clients’ mental health needs upon admission.
• Leading individual and group therapy sessions that include psychoeducational and process-oriented interventions tailored to patient needs.
• Completing discharge sessions that focus on reinforcing coping skills, outlining transition plans, and ensuring that patients and their families understand available resources.
• Coordinating with referral sources and outpatient providers throughout the patient journey to maintain continuity of care.
• Facilitating patient transfers to higher levels of care when necessary and ensuring a smooth transition.
• Engaging in crisis management within therapy sessions and providing guidance to staff during challenging interactions with patients.
• Beginning discharge planning on the day of admission to enhance the quality of care and expedite the transition process.
• Working collaboratively with multidisciplinary teams to ensure exceptional clinical standards are maintained.
• Documenting interactions and maintaining accurate records in compliance with organizational policies, including initial assessments, treatment plans, and case notes.
• Communicating effectively with the Utilization Review and Business Departments regarding authorization and billing processes.

To be considered for this role, candidates must meet the following requirements:

• A Master’s degree in Social Work, Psychology, Counseling, or a related mental health discipline is required.
• Current and unrestricted licensure as a Licensed Clinical Social Worker (LCSW).
• Proven ability to establish and model appropriate boundaries with individuals and families.
• Strong organizational skills with a commitment to timely documentation of services.
• Experience working with a culturally diverse population.
• Proficient in Office 365 and Electronic Health Record systems to facilitate efficient documentation and communication.
• Excellent verbal and written communication skills to ensure clear interactions with clients and team members.

While not required, the following experiences are preferred:

• Demonstrated ability to engage in community outreach and program promotion.
• Previous experience in crisis intervention and management within a therapeutic environment.
• Familiarity with telehealth practices and remote counseling techniques.
• Experience with developing individualized treatment plans and SMART goals.
