• Act as a vital liaison between clients, families, and outpatient providers to ensure seamless coordination of care.
• Engage with insurance companies to facilitate pre-certification and manage utilization review processes, ensuring appropriate access to services for clients.
• Contribute to discharge planning and aftercare strategies, ensuring clients have the necessary support and resources post-treatment.
• Provide therapeutic meal support, modeling healthy relationships with food and assisting clients in addressing food-related challenges.
• Deliver empathetic, affirming care tailored to individual client circumstances and their unique intersecting identities, fostering an inclusive space for healing.

This role focuses primarily on older adults who may be dealing with complex health and emotional challenges, including chronic illnesses, mental health conditions, or end-of-life issues. • A Master's degree in Social Work from an accredited institution is required.
