As a part of our Hospice Center team, you will work closely with a network of skilled professionals dedicated to providing high-quality care to patients in need of support. Key Responsibilities
  • Conduct thorough assessments of clientsโ€™ counseling needs, considering their mental health and substance use history.
  • Utilize the DSM (Diagnostic and Statistical Manual) criteria to evaluate clients for substance abuse diagnoses, ensuring accurate identification of the appropriate level of care based on ASAM (American Society of Addiction Medicine) guidelines.
  • Prepare individualized treatment plans with a specific focus on substance abuse counseling, setting goals and strategies tailored to each clientโ€™s needs.
  • Collaborate with the Clinical Director and Executive Director to create and maintain a supportive and therapeutic environment conducive to recovery.
  • Develop and document comprehensive discharge and continuum of care plans, ensuring that clients are transitioned appropriately to the next levels of care.
  • Maintain detailed clinical records documenting all counseling sessions, educational services, assessments, and follow-ups, adhering to ethical and legal standards.
  • Manage a caseload efficiently, including conducting regular case management and utilization reviews, alongside treatment planning and clinical charting as required.
  • Coordinate support services for clients and their families, ensuring that the services provided are accurately recorded in the clientโ€™s clinical record.
  • Establish contact and maintain relationships with external referral sources, including legal, educational, and employment entities to enhance client support.
  • Facilitate crisis intervention strategies for clients facing critical situations by leveraging a multidisciplinary team approach.
Patient Population / Client Focus

In this role, you will work primarily with adults struggling with co-occurring mental health and substance use disorders. You will support clients who come from diverse backgrounds and present a wide range of psychosocial issues, requiring sensitivity, cultural competence, and personalized care strategies.

Qualifications
  • Master's degree in social work (MSW) from an accredited program is required.
  • Valid LCSW licensure in the state of Illinois.
  • Proven experience in clinical social work, particularly with substance use disorders and co-occurring mental health issues.
  • Strong understanding of DSM and ASAM criteria for proper client assessment and treatment planning.
  • Excellent communication skills, both verbally and in writing; ability to document clinical work effectively.
  • Demonstrated ability to work independently and as part of a team, showing strong organizational and time management skills.
Preferred Experience
  • A minimum of two years of experience in a similar role within addiction treatment settings is preferred.
