This position allows for complete remote work, enabling flexibility while maintaining a strong connection to our patients and team members. As a fully remote team operating within a hospice center framework, you will work collaboratively with interdisciplinary teams to ensure that our patients receive comprehensive, empathetic care that addresses their emotional and psychological needs during critical times.

• Conduct timely wellness checks to assess the mental health and emotional wellbeing of clients, ensuring they feel supported and understood.
• Maintain strict adherence to all facility and licensing standards, guaranteeing quality of care and compliance with regulatory requirements.
• Provide peer support and clinical assistance to fellow team members, stepping in to cover during absences and facilitating cohesive team operations.
• Engage proactively with the Utilization Review and Business Departments to manage service authorizations and payment processes effectively.
• Guide clients in accessing community resources, helping them navigate available services for additional support with their mental health journey.
• Document all interactions and services provided accurately and expediently, ensuring continuity of care and compliance with organizational protocols.

Your primary role will be to serve a diverse demographic, focusing on both children and adults facing end-of-life issues and their families. By providing therapeutic support tailored to client needs and their unique circumstances, you will play a crucial role in enhancing their quality of life during this challenging time. Skills in multi-lingual communication and cultural competency will be vital in effectively serving our diverse client base.

• A Master’s degree in Social Work from an accredited institution.
• Active California LCSW licensure is a must.

• Demonstrated ability to meet deadlines consistently, ensuring timely documentation submission and adherence to care plans.
