• Conduct clinical evaluations to understand the psychosocial needs of patients and develop tailored counseling strategies.
• Collaborate with psychiatrists to monitor medication side effects and adjust treatment plans accordingly for optimal patient outcomes.
• Manage a diverse caseload, ensuring effective documentation as per company policies, including biopsychosocial assessments and treatment planning.
• Engage in crisis intervention when necessary, providing timely support to patients and families in challenging situations.
• Develop and implement comprehensive aftercare plans for clients upon discharge, aimed at facilitating a smooth transition and continued support post-hospice.
• Participate in treatment team meetings, engaging in utilization reviews and contributing to case management discussions.
• Advocate for patients by researching and connecting them to necessary community resources, enhancing their quality of life.

• A Master’s Degree in Social Work from an accredited graduate program is a prerequisite for this role.
