Although this role primarily allows for remote work, occasional visits to our hospice center in Joliet may be required for team meetings or training sessions.

• Conduct thorough assessments to understand clients' mental health needs, including their emotional, spiritual, and psychological distress related to terminal illness.
• Develop comprehensive treatment plans tailored to the individual needs of each client and regularly review these plans based on client progress and feedback.
• Facilitate individual and family counseling sessions using evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT) and mindfulness practices.
• Engage in crisis intervention during high-stress situations, assessing risk factors and providing immediate support to clients in distress.
• Monitor and document client progression towards their treatment goals in accordance with federal and state regulations, ensuring compliance and maintaining accurate electronic health records.
• Collaborate regularly with a multidisciplinary team of healthcare professionals, including nurses, physicians, and chaplains, to ensure comprehensive care is provided.
• Stay informed about the latest research, trends, and best practices in hospice and palliative care to enhance service delivery and client outcomes.

The primary focus of this position is on clients who are experiencing the psychological impact of terminal illness, end-of-life decisions, and complex emotional responses associated with grief and loss. Our ideal candidate will demonstrate sensitivity to clients' diverse backgrounds and uphold respect for their unique experiences during this critical stage of life.

• A Master's degree in Social Work (MSW) from an accredited institution, with a focus on clinical practice.
• A current and valid state license (LCSW) to practice clinical social work in Illinois.
• Strong communication, empathy, and interpersonal skills to establish rapport with clients and their families.
• Ability to work independently while contributing to a collaborative team environment.
• Commitment to upholding confidentiality and managing sensitive information discreetly.

• Previous experience working in hospice, palliative care, or with terminally ill clients is highly preferred.
