This role is 100% remote, allowing for flexible work arrangements while maintaining a focus on patient-centered care. As an employee of a hospice center, you will engage virtually with clients, providing essential support and therapy from the comfort of your home or designated work space. As a Licensed Clinical Social Worker, you will have a multifaceted role that involves:

• Conducting thorough assessments to determine the counseling needs of clients, including evaluations for substance use disorders based on DSM criteria.
• Developing individualized treatment plans that detail substance use counseling strategies, ensuring alignment with each client's unique needs and circumstances.
• Creating and implementing comprehensive discharge and continuity of care plans to facilitate a smooth transition for clients moving to different levels of care.
• Documenting all clinical interactions, including counseling services, assessments, referrals, and follow-ups, in accordance with HIPAA regulations to ensure privacy and confidentiality.
• Maintaining a manageable caseload, inclusive of regular utilization reviews and treatment planning duties, while ensuring that all clinical charts are up to date and compliant with organizational standards.
• Coordinating support services for clients and their families, ensuring they receive the resources and assistance needed for their journey toward recovery.
• Engaging with referral sources such as legal representatives, educational institutions, and employers to facilitate a holistic support network for clients.
• Facilitating crisis interventions using a multidisciplinary team approach, ensuring swift response to clients in urgent need.

This position primarily serves clients in hospice care, many of whom may also be battling substance use challenges. You will work closely with patients and their families, addressing not only their mental health needs but also providing emotional support during sensitive and challenging life transitions. Your role will be crucial in fostering environments of long-term recovery and peace for individuals facing terminal illness.

To be considered for this role, candidates must possess:

• A valid LCSW license and a Bachelor's Degree in Social Work or a related field.
• Strong professionalism, cultural competency, and therapeutic boundaries.
• Self-motivation and organizational skills to manage a caseload effectively in a remote environment.
• The ability to work collaboratively within a multidisciplinary team and maintain effective communication with clients, their families, and external stakeholders.

While not mandatory, the following experiences would enhance a candidate's application:

• Experience in hospice care or palliative services.
• Knowledge of substance use assessment and treatment methodologies.
• Familiarity with documentation standards in a remote care setting.
• Proficiency with telehealth systems and electronic health record (EHR) management.
