As part of the Hospice Center team, you will collaborate daily with a dedicated group of professionals, including the Clinical Director and Executive Director, ensuring a cohesive approach to treatment and resident care.

Key Responsibilities

Your responsibilities as an LCSW will include:

  • Conducting comprehensive assessments of clientsโ€™ counseling needs, understanding their unique backgrounds and challenges.
  • Utilizing DSM (Diagnostic and Statistical Manual) criteria for substance abuse diagnosis, ensuring tailored care based on the ASAM (American Society of Addiction Medicine) level of care standards.
  • Developing personalized substance abuse counseling segments for treatment plans, aligning with clientsโ€™ specific goals and needs.
  • Creating and executing client discharge and continuum of care plans that facilitate smooth transitions to appropriate care levels.
