Key Responsibilities

  • Initiate discharge planning on the day of admission to ensure continuity of care, preparing transition plans that include coping strategies and available resources.
  • Confirm discharge appointments with patients and their loved ones, ensuring they are prepared and supported as they transition back into the community.
  • Facilitate communication with referral sources and outpatient providers to maintain continuity of care throughout the treatment process.
  • Manage crises effectively during therapy sessions, utilizing therapeutic techniques and interventions to stabilize patient conditions.
  • Support the milieu staff by offering clinical insights on patient interactions, enhancing the overall therapeutic environment for clients.
  • Participate actively in clinical outreach efforts, promoting the services offered by the clinic to potential clients and community partners.
