• Act as a primary liaison between clients, their families, and outpatient providers, facilitating clear communication and effective care coordination.
• Interact with insurance providers for pre-certifications and utilization management, ensuring that clients receive timely access to necessary therapeutic resources.
• Participate in discharge planning and aftercare processes, collaborating with healthcare teams to develop comprehensive aftercare plans that address clients’ ongoing needs.
• Offer therapeutic meal support, encouraging clients to build a healthy relationship with food by promoting balanced nutritional choices and mindful eating practices.
• Deliver empathetic and affirming care tailored to the unique circumstances of each client, recognizing and integrating their intersecting identities into the therapeutic process.

This role focuses primarily on older adults receiving hospice care, encompassing a range of emotional and psychological needs associated with end-of-life issues. You will work closely with clients facing significant transitions in their health and lifestyle, providing support that respects their dignity and promotes their quality of life during this deeply personal journey.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Kansas is mandatory.
• Applicants must possess demonstrable experience working with older adults and in hospice or similar settings.
• Proficiency in clinical assessment and diagnostic skills is essential for effective client interaction.
• Strong communication and interpersonal skills are crucial to build rapport with clients, families, and interdisciplinary teams.
• The ability to work independently while also collaborating with a team environment is a key prerequisite for this role.

• Experience in hospice care, particularly with older adult populations, is highly preferred.
• A background in therapeutic meal planning or nutrition counseling would be beneficial.
• Skills in crisis intervention and conflict resolution, particularly in high-stress situations, are advantageous.
• Familiarity with insurance processes and healthcare management software is a plus.
