• Act as a liaison between clients’ families and outpatient providers, ensuring seamless communication and coordinated care.
• Engage with insurance providers to obtain necessary pre-certifications and facilitate utilization management, ensuring clients have the coverage they need for their mental health services.
• Participate actively in discharge and aftercare planning, employing a systematic approach to support clients’ transitions out of crisis.
• Provide therapeutic meal support, helping clients cultivate a healthy relationship with food while modeling positive behaviors.
• Deliver empathetic, client-centered, and affirming care tailored to each client's unique circumstances and diverse intersecting identities.

In this role, you will primarily work with older adults in a hospice setting, addressing their mental health needs as they navigate end-of-life issues. You will focus on providing holistic support that honors their life experiences while addressing emotional and psychological challenges during this sensitive time.

• A Master’s degree in Social Work from an accredited institution, equipping you with the theoretical knowledge and practical skills essential for this position.
• Possession of a valid LCSW license in the state of New York, demonstrating your qualification to practice independently.
• At least one year of relevant experience working with older adults, showcasing your ability to connect with this specific population.
• Proficient clinical and diagnostic skills, allowing for effective assessment and intervention in mental health cases.
• Strong communication and interpersonal skills to foster collaborative relationships with clients, their families, and interdisciplinary teams.
• Ability to work autonomously and within a team dynamic, demonstrating adaptability and commitment to our organizational values.

• Experience in hospice or palliative care settings, providing insight into the unique challenges faced by clients and their families during end-of-life processes.
• Familiarity with electronic health record (EHR) systems for documentation and progress tracking, ensuring continuity of care and compliance with healthcare regulations.
• Training in trauma-informed care or similar methodologies, illustrating an understanding of the complexities surrounding mental health and emotional trauma.

The salary range for this position is competitively set between USD 80,000 to 100,000 per year, commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package, including:
