You will work closely with the Clinical Director and Executive Director to foster a therapeutic environment geared towards long-term recovery. Key ResponsibilitiesAs an LCSW, you will be tasked with the following vital responsibilities:
- Conduct comprehensive assessments to identify the counseling needs of clients.
- Utilize DSM (Diagnostic and Statistical Manual) criteria to assess substance abuse diagnoses and create personalized treatment plans based on ASAM (American Society of Addiction Medicine) standards.
- Prepare detailed substance abuse counseling components for client treatment plans, ensuring they are tailored to each individual’s circumstances.
- Develop discharge and continuum of care plans that facilitate smooth transitions to subsequent levels of care.
- Document all counseling sessions, educational services, assessments, and follow-ups accurately within the client's clinical record.
- Maintain an organized caseload, including regular updates for case management and utilization review, as well as treatment planning and clinical charting.
- Coordinate support services for clients and their families, ensuring that all provided services are recorded for future reference.
- Establish strong communication lines with referral sources, including legal entities, educational institutions, and employers to support client recovery.
- Engage in crisis intervention strategies employing a multidisciplinary approach tailored to the needs of assigned clients.
The population you will serve includes individuals facing various mental health challenges, particularly those with co-occurring disorders involving substance use. QualificationsThe successful applicant must possess the following qualifications:
