• Conduct in-depth assessments of the psychosocial needs and mental health concerns of senior clients, taking into account their emotional, psychological, and social factors.
• Develop and implement individualized care plans that cater to the specific goals and preferences of clients and their families, regularly reviewing and adjusting these plans as necessary.
• Provide comprehensive counseling services to seniors and their families, addressing various life transitions and challenges they may encounter, such as bereavement, health crises, and family dynamics.
• Advocate for clients’ rights, ensuring they have access to necessary community resources such as geriatric care, mental health services, and support networks.
• Collaborate effectively with an interdisciplinary team, including healthcare professionals, caregivers, and case managers, to enhance coordinated care delivery for our clients.
• Conduct regular check-ins with clients via telehealth, assessing their overall well-being, living conditions, and any potential safety concerns.
• Educate clients and families about available community services, support groups, and programs designed to enhance social engagement and facilitate improved quality of life.
• Maintain up-to-date and accurate records of client assessments, treatment plans, and progress notes to ensure compliance with state and federal regulations.
• Stay informed and compliant with evolving social work practices, ethical standards, and relevant legislation that affects the provision of services.

The primary clientele for this position includes seniors who may face various mental health challenges and psychosocial stressors. With an understanding that seniors often grapple with issues such as loneliness, anxiety, depression, and health-related transitions, your role will be to provide compassionate and personalized support that empowers them to achieve better well-being.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW-C).
• A minimum of two years of experience in a social work role with a focus on geriatrics, behavioral health, or community services.
• Strong understanding of social work principles, aging issues, and community resources available for seniors.
• Excellent counseling, interpersonal, and problem-solving skills, with a capacity for demonstrating empathy and building trust with clients.
