• Conduct comprehensive assessments to identify the unique needs of each client, allowing for the development of tailored treatment plans.
• Facilitate individual and group therapy sessions focused on empowering clients to overcome dependencies and foster positive personal change.
• Regularly evaluate treatment goals in collaboration with clients, ensuring they align with ethical guidelines and legal standards.
• Maintain detailed and accurate progress notes and patient records in accordance with state and federal regulations, ensuring all documentation is up to date.
• Collaborate with clinical directors for chart audits and review processes to ensure the highest quality of care.
• Engage in staff meetings and clinical supervision sessions to foster teamwork and maintain high standards of operational conduct.
• Participate in quality improvement initiatives aimed at enhancing service delivery and client outcomes.
• Coordinate care with other healthcare professionals, including psychiatrists and nurses, to deliver comprehensive treatment that addresses co-occurring physical and mental health needs.

Your primary focus will be on individuals dealing with mental health disorders and substance use issues, particularly those in hospice care. You will play a vital role in supporting clients through critical moments of their recovery journeys, ensuring that they feel empowered and validated. Your work will directly impact client outcomes, helping them find stability and strength during challenging times.

• Master’s degree from an accredited institution in Social Work, Psychology, Counseling, or a related field.
• Active LCSW licensure is mandatory.
• A minimum of 2 years of direct clinical experience within social work, preferably in substance use or mental health settings.
• Demonstrable clinical and case management skills, alongside a thorough understanding of mental health treatment methodologies.
• Proficient computer skills necessary for maintaining records and documentation.
• Exceptional written and verbal communication skills to foster effective interactions with clients and team members.

• Experience working in hospice settings or with individuals facing end-of-life challenges.
• Knowledge of current evidence-based practices for mental health and substance use treatment.
• Ability to implement trauma-informed care strategies.

The salary range for this position is competitive, between USD 120,000 to 130,000 yearly, commensurate with experience.
