Key Responsibilities
  • Conduct comprehensive assessments and develop personalized treatment plans tailored to each client's unique needs. This includes creating pre-crisis and crisis management plans, as well as behavioral guidelines and risk assessments.
  • Maintain meticulous and timely documentation in accordance with regulatory requirements, ensuring that all activities are recorded accurately to enhance patient care.
  • Utilize evidence-based screening tools in collaboration with the clinical team to effectively evaluate and guide treatment strategies.
  • Collaborate with interdisciplinary teams to create and implement positive behavior support plans across various programs.
  • Actively participate in licensing and accreditation processes, contributing to the clinic's compliance and quality improvement initiatives.
  • Serve as a liaison to local and state service systems, fostering partnerships with community organizations and agencies to promote access to necessary resources.
  • Engage in open communication with clients, team members, and external stakeholders to fulfill contract standards and uphold the clinic's mission.
  • Develop and maintain relationships with various agencies and organizations to identify new alliance opportunities and funding sources.
  • Participate in ongoing professional development by attending relevant conferences, workshops, and meetings, enhancing both personal and professional acumen.
  • Embody and advocate for the core mission, values, and principles of Grace Hospital in all professional interactions.
  • Complete any additional tasks as assigned by the Clinic Director that align with your expertise and the needs of the clinic.
Patient Population / Client Focus

You will be working primarily with adults and adolescents facing a range of mental health challenges, including anxiety, depression, trauma, and substance use disorders. Our patient-focused approach emphasizes individualized care tailored to the specific needs of each client, facilitating their journey towards mental wellness and recovery.

Qualifications
  • A Master’s Degree in a behavioral health field is required, ensuring a solid foundation in therapeutic principles and practices.
  • An active, independent LMHC license from the state of Virginia, demonstrating your qualification and compliance with state regulations.
  • Strong ability to maintain clinical records and prepare detailed reports reflecting patient progress and interventions.
  • Proficiency in working effectively within an interdisciplinary team environment, promoting collaborative care.
  • Excellent communication skills to articulate ideas, treatment plans, and progress to both clients and colleagues.
  • Demonstrated judgment and decision-making ability based on professional knowledge and experience.
Preferred Experience
