Key Responsibilities
  • Perform comprehensive assessments to evaluate patient status and apply the appropriate diagnostic tools, including the DSM-5, when necessary, to ensure precise treatment planning.
  • Create and continually revise individualized care plans that respect cultural differences and accommodate specific patient needs, emphasizing a collaborative approach involving family and caregivers.
  • Monitor patient progress regularly, employing evidence-based interventions and adaptive strategies tailored to the mental health diagnoses, in coordination with the clinical team.
  • Document treatment records meticulously and adhere to regulatory and organizational guidelines using the latest electronic health record (EHR) systems.
  • Facilitate referrals and establish strong connections with community resources and other treatment providers, enhancing the continuum of care.
  • Engage in educational activities and mentorship for trainees and volunteers, contributing to their professional growth and enhancing clinical skills within the team.
  • Participate in peer reviews, quality audits, and collaborative discussions under the guidance of clinical supervisors, supporting the development of a high-performing clinical team.
Patient Population / Client Focus

Qualifications
  • A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits
