The Remote Oncology/Hematology Registered Nurse provides telephonic and remote care coordination services, assessing patient needs and collaborating with qualified staff to ensure appropriate, timely care. This role involves coordinating with the broader care team, documenting all care elements in the patient's medical record, accurately identifying high-risk conditions, proactively supporting patients through the care coordination, patient education, and directing additional care to appropriate resources as necessary. Under general supervision, the nurse delivers professional nursing care adhering to national and organizational standards and guidelines, as well as state licensing board scope of practice. The candidate must recognize the physical, psychological, and spiritual aspects of patient care and participate in company-wide quality initiatives. This pivotal role supports the US Oncology Compliance Program, including the Code of Ethics and Business Standards, to ensure patients receive timely and expert guidance, enhancing patient outcomes and satisfaction.

Key Responsibilities

  • Provide ongoing patient support across the cancer care continuum, serving as a consistent point of contact for patients and caregivers.

  • Coordinate care across the multidisciplinary team, ensuring timely access to diagnostics, treatments, and supportive services.

  • Identify and address barriers to care (clinical, financial, logistical, and psychosocial), connecting patients with appropriate resources.

  • Conduct telephonic assessments of patients, determining the necessary level of care and facilitating outpatient and/or telehealth visits when symptoms can be managed in that setting.

  • Educate patients on symptom management, medication side effects, and treatment plans, providing clear instructions and empathetic support to enhance their understanding and compliance.

  • Collaborate with other members of the healthcare team to ensure comprehensive patient care, documenting all interactions meticulously in the electronic health record (EHR) system.

  • Utilize and adapt quickly to various technology and reporting platforms to ensure efficient and accurate remote patient care and documentation.

  • Identify potential barriers to care, assisting patients in navigating healthcare systems and resources.

  • Participate in ongoing professional development and stay current with best practices in oncology nursing and telephonic triage.

Schedule

Working hours of 7am-4pm Central time, Monday - Friday.

Minimum Requirement

Degree or equivalent and typically requires 2+ years of relevant experience.

Certification

Active and unrestricted Registered Nurse (RN) Texas multistate or other compact state license (additional state licenses may be required).

Critical Skills

  • Oncology Certified Nurse (or required to obtain within 1 year).

  • Minimum of 2 years of clinical nursing experience, with recent years in oncology (Heme/Onc clinic or hospital experience specifically).

  • Excellent communication and interpersonal skills.

  • Strong critical thinking and problem-solving abilities.

  • Proficiency in using electronic health record systems and telecommunication tools.

  • Ability to work independently and manage time effectively in a remote environment.

  • Compassionate, patient-centered approach to care.

  • Attend all onsite meetings and trainings located in The Woodlands, Tx office.

Preferred

  • Bachelor's degree in Nursing (BSN) preferred.

  • Experience in telephonic triage or remote patient care is strongly preferred.
