As the Licensed Clinical Social Worker, you will engage in the following key responsibilities:

• Maintain accurate and contemporaneous clinical documentation, including progress notes and treatment plans, in accordance with department policy.
• Collaborate closely with a multidisciplinary healthcare team to develop and implement individualized treatment plans based on thorough psychosocial assessments and evidence-based social work interventions.
• Consult and coordinate with patients' families or support systems to ensure comprehensive understanding of treatment objectives and discharge planning.
• Create tailored treatment plans that address specific patient circumstances, focusing on their psychological and emotional needs.
• Conduct regular counseling sessions, using a variety of therapeutic modalities to support patients in managing their emotions and behaviors.
• Perform ongoing assessments of patient progress, adjusting treatment strategies proactively to ensure optimal outcomes.

Your work will primarily focus on patients facing terminal illnesses, their families, and caregivers. You will provide emotional support and counseling, helping patients navigate the complex feelings associated with end-of-life care. Your contributions will be essential in promoting psychological resilience and facilitating a dignified transition.

The ideal candidate must possess the following qualifications:

• A Master’s degree in Counseling, Social Work, or a related field from an accredited institution.
• Licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
• A minimum of three years of professional experience in the field of social work, preferably within a hospice or palliative care environment.
• Current Associate or Licensed status with the Board of Behavioral Sciences (or in progress).
• Exceptional organizational skills and the ability to manage multiple priorities in a fast-paced environment.
• A self-motivated approach to work, demonstrating the initiative in documenting billing hours and maintaining accurate records.

While not mandatory, candidates with the following experience will be preferred:

• Previous experience working with terminally ill patients and their families.
• Familiarity with hospice care frameworks and multidisciplinary team dynamics.
• Proficiency in electronic health record (EHR) systems, which can streamline documentation and patient interaction.
