• Conduct thorough assessments of clients to understand their mental health needs, history, and treatment objectives.

• Develop, implement, and monitor personalized treatment plans tailored to each client's unique circumstances and goals.

• Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in a clinical mental health setting, ideally with experience in hospice or palliative care.
• Strong clinical skills in assessment, treatment planning, and the application of therapeutic interventions.
• Exceptional communication and interpersonal skills, facilitating empathetic client engagement.

• Experience with evidence-based treatment modalities specific to grief and loss.
• Understanding of hospice and palliative care principles and methodologies.
• Previous background working within a remote or telehealth framework.
