Key Responsibilities
  • Act as a liaison between clients, families, and outpatient service providers to ensure effective communication and care coordination.
  • Engage with insurance companies for pre-certifications and utilization management, advocating for client needs and treatment plans.
  • Participate in discharge and aftercare planning, streamlining transitions for clients returning to their community and ensuring follow-up support.
  • Provide therapeutic meal support which includes facilitating mindful eating practices and developing a healthy relationship with food.
  • Deliver empathetic, affirming care that respects each client's unique identity and circumstance, focusing on tailored interventions.
Patient Population / Client Focus

The primary focus of this role is on older adults who may be experiencing a range of mental health challenges, including depression, anxiety, and adjustment disorders. You will work closely with clients to develop individualized treatment plans that address their specific needs and circumstances. Your expertise will ensure that all clients receive meaningful emotional support that promotes overall well-being.

Qualifications
  • A Master's degree in Social Work from an accredited institution, ensuring a strong foundational knowledge in clinical social work practices.
  • A valid LCSW license in the state of Texas is essential for this role.

Compensation and Benefits
