• Conduct comprehensive clinical assessments to evaluate patients’ mental health status and identify their specific needs.
• Develop individualized treatment plans using evidence-based practices and solution-focused interventions.
• Administer therapeutic interventions that are tailored to each patient’s unique situation, ensuring an effective approach to treatment.
• Document clinical progress and treatment outcomes accurately and in a timely manner, adhering to regulatory requirements and organizational policies.
• Engage in collaborative discussions with a multidisciplinary team, enhancing care coordination and comprehensive treatment planning.
• Participate in case consultations and peer review meetings to promote quality improvement and professional development.

In this role, you will work with a diverse array of clients, including individuals coping with anxiety, depression, trauma, and various mental health disorders. Your expertise will help guide patients in their treatment journeys, facilitating their progression toward achieving personal treatment goals and improving their overall quality of life.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Tennessee.
• A minimum of three years of clinical experience specializing in psychotherapy and mental health treatment.
• Proficiency in using solution-focused psychotherapy interventions.
• Exceptional communication skills, both verbal and written, to effectively engage with clients and share insights with colleagues.

• Previous experience in a hospice or palliative care setting is highly desirable.
