• Conduct comprehensive assessments and clinical evaluations for clients suffering from major mental illnesses and acute emotional disorders.
• Collaboratively create, implement, and monitor integrated care plans while ensuring smooth communication between clients, their families, and other healthcare providers.
• Engage actively in client sessions, offering evidence-based short-term interventions for treatment, prevention, and recovery tailored to individual needs.
• Participate in on-call rotations for crisis intervention, providing timely and effective responses to client emergencies.
• Maintain accurate case documentation, ensuring all reports and data submissions are completed in a timely manner, meeting organizational and regulatory standards.
• Assist with coordinating care transitions and continuous monitoring of client status to ensure adherence to treatment goals.
• Contribute to strategic committees aimed at enhancing service delivery and promoting quality improvement across the organization.

The Licensed Independent Clinical Social Worker will primarily work with terminally ill patients and their families in hospice care, requiring a compassionate and understanding approach. This includes collaborating with patients from diverse backgrounds, respecting their cultural beliefs and practices. Understanding the emotional complexities associated with end-of-life care is essential to providing appropriate support and guidance.

• A Master's Degree in Social Work (MSW) from an accredited program.
• Current Licensure as an Independent Clinical Social Worker (LICSW).
• Expertise in assessment, diagnosis, and treatment methodologies relevant to assigned patient populations.
• Strong organizational, planning, and interpersonal skills promoting collaboration within diverse teams.
• Proficient in communication, both verbal and written, enabling effective interaction with patients, families, and providers.

• At least two years of clinical experience, particularly in a hospice or healthcare setting.
• Background in trauma-informed care or similar therapeutic approaches.

This position offers a competitive salary ranging from USD 70,000 to 85,000 per year, commensurate with experience.
