Key Responsibilities

  • Proactively organize and propose schedules for appointments, home visits, and observations to track patient development.
  • Coordinate and execute home visits that include in-home observations and data collection, ensuring a holistic view of the patientโ€™s environment and challenges.
  • Implement and monitor program plans effectively, adjusting strategies in response to feedback from patients and team members.
  • Actively participate in Interdisciplinary Team Meetings and contribute valuable insights regarding patient care and progress.
  • Prepare detailed monthly and quarterly progress notes to keep all stakeholders informed.
  • Provide oversight and training to staff members to ensure they are well-equipped to utilize program plans and complete documentation accurately.
  • Engage in follow-up interviews and observations to further support patient needs and enhance their treatment experience.
  • Maintain comprehensive and accurate records to ensure transparency and accessibility for all involved parties.
  • Serve as a liaison between the organization and the larger mental health community to support development goals and promote awareness.
  • Assist in developing training materials and procedures to continuously improve counseling programs and patient outcomes.
Patient Population / Client Focus

As a member of our team, you will work with a diverse client population, including adults and youth, facing a range of mental health challenges. Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a related field is required.
