• Conduct thorough assessments to tailor treatment plans that address individual patient needs.
• Regularly evaluate patient progress and communicate findings to supervisors, family members, and relevant stakeholders to ensure comprehensive support.
• Proactively organize and propose schedules for appointments, home visits, and observations to track patient development.
• Coordinate and execute home visits that include in-home observations and data collection, ensuring a holistic view of the patient’s environment and challenges.
• Implement and monitor program plans effectively, adjusting strategies in response to feedback from patients and team members.
• Actively participate in Interdisciplinary Team Meetings and contribute valuable insights regarding patient care and progress.
• Prepare detailed monthly and quarterly progress notes to keep all stakeholders informed.
• Provide oversight and training to staff members to ensure they are well-equipped to utilize program plans and complete documentation accurately.
• Engage in follow-up interviews and observations to further support patient needs and enhance their treatment experience.
• Maintain comprehensive and accurate records to ensure transparency and accessibility for all involved parties.
• Serve as a liaison between the organization and the larger mental health community to support development goals and promote awareness.
• Assist in developing training materials and procedures to continuously improve counseling programs and patient outcomes.

As a member of our team, you will work with a diverse client population, including adults and youth, facing a range of mental health challenges. Your expertise in areas such as family therapy, couples therapy, substance misuse, and trauma will be instrumental in providing personalized care that addresses the unique needs and circumstances of our patients.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Must possess active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Maryland.
• A minimum of two years of counseling experience in a clinical setting is essential.
• A robust understanding of ethical and legal standards governing counseling practices is necessary.
• Exceptional interpersonal and communication skills to build rapport and collaborate effectively with patients and team members.
• Demonstrated ability to work cohesively within a healthcare team environment.
• Proficiency in electronic medical record (EMR) software and associated technologies.

• Experience in specialized areas such as family therapy, couples therapy, substance misuse, or trauma is highly desirable.
• A proven track record of conducting patient assessments and developing treatment plans tailored to individual needs.
• Previous involvement in interdisciplinary team settings will be beneficial.

The salary for this role ranges from USD 90,000 to 100,000 annually, depending on your experience and caseload availability.
