• Conduct thorough assessments of individual patient needs, crafting personalized treatment plans that reflect patient and family goals.
• Utilize evidence-based interventions, such as Cognitive Behavioral Therapy (CBT) and mindfulness techniques, to support patient well-being throughout their hospice journey.
• Maintain meticulous and confidential clinical records, ensuring compliance with all relevant regulatory standards.
• Collaborate with a specialized interdisciplinary team, including physicians, nurses, and spiritual care providers, to offer comprehensive, holistic care that addresses every aspect of a patient’s well-being.
• Provide education and support to patients and their families about available resources and coping strategies during challenging times.
• Stay informed on the latest research and developments in mental health to continuously enhance the quality of care provided.

Your primary focus will be assisting patients in the hospice setting, who are often managing complex emotional and psychological issues related to advancing health conditions. You will need to be skilled in dealing with grief and loss, helping both patients and their families navigate the process of end-of-life care. Your expertise will be crucial in creating an environment of support and understanding during a vulnerable time.

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Kansas.
• At least 2 years of clinical experience in a mental health setting, preferably with end-of-life care.
• Proven proficiency in solution-focused therapy and related psychotherapy interventions.
• Excellent communication and interpersonal skills, with the ability to empathize and connect with clients effectively.
• Strong analytical and problem-solving capabilities, enabling critical thinking in challenging situations.
