As an integral member of our Hospice Center, you will provide crucial mental health services to individuals in a compassionate and professional manner, helping patients and families navigate the emotional complexities surrounding hospice care.

This full-time position is remote, allowing for flexible work arrangements while maintaining a focus on delivering exceptional patient care. As an LCSW at our Hospice Center, you will work closely with a multidisciplinary team of healthcare professionals, including nurses, physicians, and spiritual care providers, to ensure patients receive holistic support. You will play a key role in enhancing our patient care processes and outcomes.

• Conduct thorough mental health assessments for hospice patients and their families to identify emotional and psychological needs.
• Design and implement individualized treatment plans that address mental health challenges while promoting comfort and emotional well-being near the end of life.
• Provide evidence-based therapeutic interventions, leveraging modalities such as cognitive-behavioral therapy (CBT) and supportive counseling to assist patients with various psychological concerns.
• Facilitate individual and group therapy sessions, encouraging emotional expression and coping strategies among patients facing terminal illness.
• Collaborate with a diverse team of healthcare providers to coordinate comprehensive care, ensuring that all aspects of the patient's needs are met.
• Offer crisis intervention during emotional distress, employing de-escalation techniques, and making appropriate referrals to specialized mental health services as required.
• Educate patients and families about mental health conditions, treatment options, and available community resources to empower them during their hospice journey.
• Maintain meticulous records, including assessments, treatment plans, and progress notes, in compliance with ethical and legal standards.
• Participate in regular team meetings, case discussions, and ongoing training to enhance clinical skills and knowledge of hospice and palliative care.
• Stay informed about the latest legislation, regulations, and best practices to ensure the highest standards of ethical care within the hospice setting.

You will support not only the emotional and psychological health of patients but also provide their families with the necessary tools and resources to cope during a challenging time. The diverse backgrounds of our patients and their families will present varied emotional needs and cultural considerations that will enrich your professional experience.

• Master's degree in Social Work from an accredited program.
• Active and valid LCSW licensure in the state of New Jersey.
• A minimum of two years of post-licensure experience, preferably in healthcare, hospice, or mental health settings.
• In-depth understanding of mental health disorders and trauma-informed care approaches.
• Proficient in conducting mental health assessments and forming effective treatment plans.
• Demonstrated experience in providing individual and group therapy, applying a variety of therapeutic techniques.
• Strong crisis intervention skills, with the ability to manage tense and emotional situations effectively.
• Exceptional interpersonal capabilities, fostering trust and rapport with patients and their families.
• Sound judgment and the ability to maintain professional boundaries.
• Familiarity with electronic health records (EHR) systems and a level of computer proficiency sufficient for clinical documentation.

• Experience working in hospice care or palliative settings.
• Training in trauma-informed practices and advanced therapeutic methodologies.
• Background in working within multidisciplinary teams to improve patient care.
• Knowledge of community resources pertinent to end-of-life care.

The annual salary range for this role is USD 80,000 to 100,000, commensurate with experience and qualifications.
