• Conduct detailed psychosocial assessments focused on understanding the emotional, mental, and social factors that affect seniors.
• Design tailored care plans in collaboration with clients and their families, addressing specific goals and preferences.
• Provide counseling and emotional support to seniors and their families, facilitating their adaptation to life transitions and challenges.
• Advocate for clients' rights and access to community resources, ensuring they receive necessary support and service connections.
• Engage with interdisciplinary teams comprised of nurses, caregivers, and healthcare professionals to enhance the overall care plan for each client.
• Perform regular check-ins and home visits to monitor well-being, assess living conditions, and address safety concerns.
• Facilitate effective communication among family members involved in the care of the senior client, helping to resolve conflicts when necessary.
• Educate clients and families about local resources, support groups, and programs that foster social engagement and enhance quality of life.
• Maintain accurate documentation of client assessments, care plans, progress reports, and all interactions per ethical and regulatory standards.
• Stay updated on best practices and changes in social work policies and comply with ethical regulations.

The primary focus of this role is on the elderly, who may face a range of psychosocial issues including isolation, chronic health conditions, and difficulties related to life transitions such as loss of independence or grief. Social Workers at Clarity Pathways Counseling are positioned to provide critical support and advocacy to not just the seniors, but also their families, thus enhancing their collective functioning and emotional well-being.

• A Master’s degree in Social Work from a regionally accredited institution.
• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• At least two years of applicable experience in social work, particularly within community centers or geriatrics.
• In-depth knowledge of social work principles and frameworks, particularly concerning aging populations and relevant community resources.
• Exceptional assessment, counseling, and conflict resolution abilities.
• A demonstrated capacity for empathy, strong rapport-building, and communication with elderly clients and their families.
• Experience collaborating with interdisciplinary teams to achieve holistic care outcomes.
• Proficiency in electronic health record (EHR) systems and relevant software applications.

Experience in community outreach or geriatric social work is highly desirable. Familiarity with local resources in Erie, PA, will provide an advantage in connecting clients with the services they need. Candidates with continuing education in social work ethics or gerontology will be particularly competitive.
