Key Responsibilities
  • Conduct thorough assessments to determine the individual needs of each patient, adapting therapeutic approaches as necessary to ensure effective treatment.
  • Maintain open lines of communication with supervisors, direct care staff, family members, and guardians to provide updates on patientsโ€™ progress and coordinated care plans.
  • Develop and share tentative schedules with supervisors that outline appointments, home visits, and observations for effective monitoring of patient progress.
  • Coordinate home visits and in-home observations in accordance with established protocols and best practices, ensuring all necessary documentation is completed.
  • Participate actively in Interdisciplinary Team Meetings and contribute to case discussions aimed at providing comprehensive care.
  • Document monthly and quarterly progress notes, capturing the effectiveness of treatment plans and making necessary adjustments.
  • Facilitate training sessions to ensure that all staff are competent in implementing program plans and maintaining accurate documentation.
  • Provide follow-up support through interviews and home visits, ensuring that patients receive the assistance they need as they progress in their treatment.
  • Keep meticulous records that are easily accessible to relevant stakeholders for quality assurance and continuous improvement.
  • Act as a liaison between the center and the broader mental health community to enhance collaborative efforts and support developmental goals.
  • Contribute to the creation and refinement of training materials and procedures aimed at further enhancing the effectiveness of our programs.
Patient Population / Client Focus

You will work with a diverse group of clients, including adults and youth, who are navigating various mental health challenges.
