• Conduct comprehensive assessments and evaluations to identify clients’ mental health needs, social determinants of health, and establish effective treatment goals.
• Provide individual counseling services, addressing a spectrum of mental health issues, including anxiety, depression, grief, and adjustment disorders.
• Facilitate supportive group therapy sessions to promote peer connection, psychoeducation, and common mental health concerns.
• Develop customized treatment plans by employing evidence-based interventions and measurable outcomes tailored to each client.
• Implement crisis intervention strategies, including thorough risk assessments, safety planning, and emergency resource coordination when necessary.
• Advocate for clients’ rights and resource accessibility, guiding them through the complexities of healthcare services and community supports.
• Maintain meticulous, confidential records of client interactions, including assessments, treatment strategies, progress documentation, and outcomes, adhering to all legal and ethical standards.

This position focuses on clients receiving hospice care, encompassing a diverse range of individuals with various backgrounds and health conditions. You will work with patients facing life-limiting illnesses and their families, providing vital emotional and practical support during challenging times. The role requires sensitivity to the complexities of grief, loss, and end-of-life issues.

• A Master's degree in Social Work (MSW) from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated clinical experience in social work and counseling, particularly in hospice, healthcare, or mental health settings.
• Strong interpersonal communication skills to effectively engage with clients and their families.
• Cultural competence and the ability to work sensitively with diverse populations.

We offer a competitive annual salary ranging from USD 80,000 to 95,000, depending on experience and qualifications.
