Position Summary
The Patient Care Coordinator supports high-quality patient care by serving as a liaison between patients, physician practices, hospitals, support services, and payors. This role is responsible for coordinating sleep study scheduling, documentation, and communication to ensure accurate workflows, regulatory compliance, and a positive patient experience.
Essential Duties and Responsibilities
Patient Coordination and Scheduling
  • Receive and enter sleep study orders into designated systems, ensuring medical necessity aligns with documented symptoms and physician orders
  • Schedule patients for sleep studies in a timely manner and in accordance with company policy
  • Obtain and verify demographic and medical information during the scheduling process
  • Inform, educate, and respond to patient and physician questions regarding the sleep study process

Communication and Documentation
  • Communicate information received from patients, physician offices, and hospitals to appropriate internal teams
  • Review and verify source materials to ensure accuracy and completeness in accordance with Medicare and payer standards
  • Follow up to correct or complete missing or inaccurate information as needed

Compliance and Operational Support
  • Perform duties in compliance with HIPAA guidelines and company policies
  • Support cross-training initiatives, including coverage of other Patient Care Coordinator functions such as Medical Records, as assigned
  • Perform additional operational tasks to support departmental workflows as needed

Required Qualifications
  • Minimum of 2-4 years of experience in a customer service role
  • Prior experience in a healthcare setting or healthcare-related organization required
  • Strong organizational skills and attention to detail
  • Ability to work effectively in a fast-paced environment
  • Effective verbal and written communication skills

Preferred Qualifications
