• Conducting thorough initial assessments for new patients to establish a comprehensive understanding of their mental health needs.
• Developing personalized treatment plans informed by evidence-based practices and patient goals.
• Delivering ongoing therapeutic sessions to individuals, including teenagers, adults, and couples, focusing on mental health challenges arising from grief, trauma, and end-of-life issues.
• Monitoring patient progress and adjusting treatment plans as necessary, based on feedback and clinical observations.

This role is specifically focused on clients in hospice care and their families, dealing with profound emotional challenges as they navigate end-of-life circumstances. Your expertise will enable you to offer critical support, helping patients cope with grief, anxiety, and other mental health concerns associated with hospice care and terminal illnesses.

• A Bachelor’s degree in Counseling, Social Work, Marriage and Family Therapy, or a related field from an accredited institution.
• Current licensure as a mental health clinician (LMFT, LPC, LCSW) is required.
• Demonstrated proficiency in treatment planning and mental health counseling skills.
• Excellent communication skills, both verbal and written, to effectively engage with patients and interdisciplinary teams.
• Comfort and experience in conducting virtual appointments; familiarity with telehealth platforms is preferred.

• A minimum of 2 years of active mental health counseling experience, particularly in a remote or telehealth setting.
• Experience working with diverse populations, particularly those facing end-of-life issues or bereavement.
• Specialized training or certifications in grief counseling or trauma-informed care.
