Become an RN Case Manager with Tribute Home Care and play an important role in setting a new standard in elder care.

Tribute Home Care is seeking a customer-service focused, collaborative RN Case Manager to join our Maryland team. This is a part-time (24 hours/week), primarily remote position for an experienced Registered Nurse passionate about delivering exceptional care and building meaningful relationships with clients and families.

We do this by hiring only those caregivers who can build real connections with their clients and by offering them the community of support and recognition they deserve.

A day in the life of an RN Case Manager

In this role, you will partner with clients, families, caregivers, and healthcare professionals to coordinate care and provide an exceptional client experience.

This will be done by:

  • Providing prospective clients with a warm and helpful introduction to Tribute.
  • Managing the setup and day-to-day delivery of services for a caseload of clients.
  • Communicating promptly, thoroughly, and proactively with all parties involved in each client's care.
  • Providing back-up clinical support and supporting other Tribute markets as needed.

This position is primarily remote and requires occasional travel to the Baltimore area approximately every 6–8 weeks, as needed.

What you need to be qualified for this position

  • Associate's or Bachelor's degree in Nursing
  • Current Maryland RN license
  • At least two years of nursing experience
  • Prior case management experience, preferably in elder care
  • Unwavering, passionate commitment to the customer experience and service excellence
  • Strong writing and communication skills
  • Strong attention to detail and organization
  • Collaborative spirit
