Psychiatric Mental Health Nurse Practitioner (PMHNP) - Remote
Remote | Maryland Patient Panel | 1099 Contractor | Up to 20 Hrs/Week
This is a fully remote 1099 independent contractor opportunity serving patients in Maryland. Clinicians should have availability during standard Eastern Time business hours, with services anticipated at up to approximately 20 hours per week based on patient volume and program needs.

Avail Health is focused on improving care for patients with complex medical, behavioral health, and social needs through interdisciplinary, technology-enabled care models.
We are seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) who wants to make a meaningful difference in the lives of adult and geriatric patients, including those with complex behavioral health needs. This fully remote opportunity will primarily focus on psychiatric evaluation, medication management, and longitudinal behavioral health treatment.
As part of our Comprehensive Behavioral Health Program, the PMHNP will work closely with an interdisciplinary team providing psychiatry, psychotherapy, behavioral health care management, and dementia care. The role will also support behavioral health referrals through our Care Transitions Program, helping patients access appropriate psychiatric care following hospitalization.
For paneled patients receiving dementia care through the CMS Guiding an Improved Dementia Experience (GUIDE) Model, the PMHNP will serve as the GUIDE Practitioner and collaborate with the interdisciplinary team to support patients and their caregivers.

What You'll DoBehavioral Health & Dementia Care
The primary focus of this role is providing longitudinal psychiatric care and clinical oversight for patients enrolled in Avail Health's Comprehensive Behavioral Health Program.
  • Conduct comprehensive psychiatric evaluations and provide diagnosis, treatment, and medication management for adult and geriatric patients with a range of behavioral health needs, including complex and co-occurring conditions.
  • Develop individualized, evidence-based treatment plans and provide ongoing psychiatric care for a longitudinal behavioral health patient panel.
  • Provide clinical oversight of behavioral health and dementia care management plans, working closely with care managers to assess patient progress, address changes in clinical needs, and guide ongoing care.
  • Collaborate with the interdisciplinary care team, including RNs, SWs, and Care Navigators, to coordinate behavioral health, medical, functional, and psychosocial needs and support comprehensive patient care.
  • Participate in interdisciplinary case reviews and clinical care planning.
  • Serve as the GUIDE Practitioner for assigned paneled patients participating in the CMS GUIDE Model, including clinical oversight of individualized dementia care plans and collaboration with the interdisciplinary team to support patients and caregivers.
Care Transitions Support
The PMHNP will also support Avail Health's Care Transitions Program, providing post-hospital care with a particular focus on patients with behavioral health needs and supporting continuity into appropriate longitudinal services.
  • Evaluate and manage behavioral health needs identified during the post-hospital transition, including psychiatric evaluation, treatment, and medication management as clinically appropriate.
  • Collaborate with RNs, SWs, Care Navigators, and other members of the Care Transitions team to identify patient needs, coordinate interventions and follow-up, and support safe transitions after hospitalization.
  • Facilitate continuity into longitudinal behavioral health and other appropriate care management programs when ongoing support is indicated.
  • Based on qualifications and program needs, provide cross-coverage for broader Transitional Care Management (TCM) services, including post-discharge evaluations, medication reconciliation, medical decision-making, individualized care planning, and clinical management throughout the TCM episode.
  • Provide TCM and other medical cross-coverage only within the practitioner's education, certification, clinical competency, licensure, and granted privileges.
What We're Looking For
Required Qualifications
  • Current board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Current, active, unrestricted Maryland APRN/NP license.
  • Current DEA registration and ability to maintain all licenses, certifications, and registrations required to provide services.
  • Experience providing psychiatric evaluation, diagnosis, treatment, and medication management.
  • Experience caring for adult and/or geriatric patients with behavioral health needs.
  • Ability to successfully complete applicable credentialing, privileging, payer enrollment, background screening, compliance training, and onboarding requirements.
  • Ability to provide care effectively within a fully remote, technology-enabled clinical environment.
Strongly Preferred
  • Dual board certification as a PMHNP and in a primary care NP specialty, such as Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
  • Active NP licensure in one or more additional states where Avail Health provides or is expanding services, particularly California, Ohio, Indiana, and West Virginia.
  • Existing LLC or other established business entity and interest in providing services through a 1099 independent contractor arrangement.
  • Experience caring for patients with complex behavioral health needs, dementia, cognitive impairment, and co-occurring medical conditions.
  • Experience with integrated behavioral health, dementia care, behavioral health care management, transitional care, post-acute care, or value-based healthcare.
  • Experience collaborating within interdisciplinary care models.
Schedule & Engagement
This is a fully remote 1099 independent contractor opportunity, with services anticipated at up to approximately 20 hours per week based on patient volume and program needs. The primary focus will be Comprehensive Behavioral Health services.
We are particularly interested in clinicians who are already established as independent practitioners through their own LLC or other business entity and are interested in growing their patient panel alongside Avail Health's programs.
As patient volume and programs grow, opportunities for expanded services and additional hours may become available. Avail Health may also consider a future W-2 employment opportunity based on organizational needs and mutual interest; future employment is not guaranteed.
Compensation & Support
  • Clinical services: $85/hour
  • Designated non-clinical services: $50/hour

Avail Health facilitates applicable credentialing, privileging, and payer enrollment and provides access to required clinical technology and Company-issued equipment, as applicable, to support secure and compliant delivery of services.
