• Conduct comprehensive assessments to evaluate the psychosocial needs of senior clients, focusing on emotional, mental, and social factors that impact their well-being.
• Collaborate closely with clients and their families to develop customized care plans that reflect their unique goals, preferences, and life situations.
• Provide compassionate counseling and emotional support, assisting seniors in navigating transitions and addressing challenges in their lives.
• Act as an advocate for clients, ensuring their rights are upheld and that they have access to essential community resources and services.
• Work collaboratively with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate and enhance the care experience for each client.

• Maintain accurate and timely documentation of client assessments, care plans, progress notes, and interactions to ensure compliance and continuity of care.
• Stay current with social work practices, policies, and regulations to uphold ethical standards in all service delivery.

• Master's degree in Social Work from an accredited institution.
