This role operates in a fully remote setting, allowing for flexibility in your work schedule while maintaining a collaborative team environment. You will have regular interactions with healthcare professionals, including doctors, nurses, and other social workers, using digital communication platforms to ensure the delivery of integrated care. • Assessment and Planning: Conduct comprehensive assessments of patients' psychosocial conditions by gathering data through effective interviewing techniques, reviewing charts, and consulting with the healthcare team. Formulate tailored treatment plans based on thorough evaluations.
• Therapeutic Techniques: Implement evidence-based practices, including behavior modification and assertiveness training, to optimize patient engagement and emotional resilience. Provide supportive therapy that aligns with individual patient needs.
• Documentation: Maintain accurate and timely documentation of patient goals, treatment plans, and responses to therapeutic interventions, ensuring compliance with regulatory and organizational standards.
• Group Facilitation: Lead educational and therapeutic groups aimed at improving social skills, emotional regulation, and coping mechanisms among patients. Monitor participants' progress and adapt group sessions as necessary to maximize therapeutic benefit.
• Collaboration: Build strategic partnerships with community services and resources to enhance support systems for patients and families in navigating their health journeys.

Your work will primarily focus on individuals and families facing various mental health challenges, such as depression, anxiety, trauma, and adverse life experiences. The patient demographic includes a diverse population within the community, encompassing different cultural and socioeconomic backgrounds. This role provides you the platform to make meaningful interventions in their lives, fostering hope and resilience.

• Master’s degree in Social Work from an accredited institution.
• Active California license as a Clinical Social Worker (LCSW).
• Demonstrated proficiency in both written and verbal communication, with the ability to articulate complex concepts in an understandable manner.
• Experience in establishing and maintaining professional boundaries with clients to promote a healthy therapeutic alliance.
• Strong organizational skills with the capacity to manage multiple cases and complete documentation promptly.

• Experience working in a community health setting or healthcare environment is highly desirable.
• Familiarity with electronic health record (EHR) systems to document patient interactions and treatment plans efficiently.
• Knowledge of cultural competency practices and experience working with diverse populations.

We offer a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and caseload.
