• Conduct thorough assessments of patients’ psychological, emotional, and social needs to develop tailored treatment plans.
• Implement evidence-based therapeutic interventions and monitor patient progress, adjusting care plans as necessary.
• Maintain accurate and confidential clinical records in compliance with state regulations and organizational protocols.
• Collaborate closely with a multi-disciplinary care team to ensure comprehensive support for patients and their families.
• Provide education and emotional support to patients and their families, guiding them in navigating healthcare decisions and coping strategies.
• Stay informed on the latest developments in mental health and hospice care practices to ensure quality and relevant patient interventions.

You will primarily work with patients facing life-limiting illnesses and their families, addressing their unique psychological and emotional challenges. Your role involves providing care to a diverse patient population, ensuring culturally competent and sensitive approaches tailored to their individual backgrounds and needs.

• Master's degree in Social Work from an accredited institution.
• A valid LCSW-C license in the state of Maryland.
• A minimum of 2 years of clinical experience, preferably in a healthcare or hospice setting.
• Proficient in solution-based psychotherapy interventions and comfortable using telehealth platforms.
• Strong communication, interpersonal, and organizational skills.
• Ability to engage effectively with a multidisciplinary team and work independently.

Experience in hospice or palliative care settings is preferred, especially familiarity with the challenges faced by patients with terminal illness. Understanding trauma-informed care approaches and having additional certifications in mental health modalities can enhance your application.

The salary for this position ranges from USD 80,000 to 100,000 annually, dependent on experience and qualifications.
