As an LCSW, your role will encompass a variety of responsibilities tailored to the unique needs of our hospice clients:

• Conduct comprehensive client assessments and evaluations to understand their psychosocial needs and develop appropriate intervention strategies.
• Utilize evidence-based behavioral intervention techniques to assist clients in coping with the emotional and psychological aspects of terminal illness.
• Facilitate individual and group counseling sessions remotely, providing a safe space for clients and families to express their concerns and feelings.
• Support families by equipping them with tools, resources, and strategies to cope with grief and loss, fostering a healing environment for all clients.
• Ensure accurate and timely clinical documentation in compliance with regulatory standards, using electronic health records systems.
• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to create comprehensive care plans that address the holistic needs of each client.

Your clients will primarily include individuals receiving hospice care, along with their families, who are often faced with complex emotional challenges. You will provide support for a wide range of issues related to grief, anxiety, depression, and adjustment to life-limiting illnesses. Your expertise will help clients navigate their emotional health while ensuring that their dignity and quality of life are prioritized during this critical time.

The ideal candidate for this role will possess the following qualifications:

• A Master’s degree in Social Work or a closely related field.
• An active LCSW license in the state of California.
• A minimum of two years of clinical experience, particularly in a hospice, palliative care, or other health service settings.
• Proficiency in Microsoft Outlook, Word, and other relevant office software.
• Experience managing electronic health records systems for maintaining accurate client data.

While the minimum requirement is two years of clinical experience, candidates with three years or more in hospice or palliative care settings may find that their application stands out. Familiarity with community resources and previous experiences in crisis intervention would be advantageous, as would any additional certifications in grief counseling or trauma-informed care.
