Join our team and make a positive impact on the lives of others! We are looking for a HomeCare Registered Nurse (RN) for our Georgia team.
This is a remote part time position in the Georgia area but may require visits to one of our closest FreedomCare Offices in Georgia. Candidate MUST reside in the state of Georgia.
Department & Position Overview:
Our program is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient's independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.
In this position, as a Part Time Registered Nurse, you will perform clinical evaluations of the member to assess the needs of the patient to ensure appropriate measures are provided for enrollment, development of service plan and ongoing compliance.
Every Day You Will:
  • Provides support to the caregiver and member, and reviews services according to scope.
  • Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency's compliance program and privacy policy.
  • Follows the FreedomCare's clinical policy and procedures.
  • Assesses and regularly reassess the client's needs and their PCP summary form, treatment and education and communicates any changes to the member's PCP.
  • Develops implements, evaluates, revises, and provides input to the client's plan of care to include the physician's plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client's care and community resources.
  • Interprets the philosophy and objectives of the agency to clients, families, and others.
  • Performs other job-related duties as may be required or directed by the supervisor.

Ideal Candidate Will Possess:
  • Registered Nurse licensed by Georgia Board of Registration in Nursing
  • Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities
  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)
  • Valid driver's license with an insured vehicle for travel within the state
  • Strong communication and interpersonal skills.

  • Organizational and multitasking skills.
  • Excellent communication, critical thinking, and organizational skills

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this per diem role is between $40.00 and $42.00 per hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
