Position Summary
Make a difference in people's lives while enjoying a flexible schedule. The Assessment RN Care Manager guides families through the assessment process, ensuring quick activation of services. This role involves evaluating eligibility, creating initial care plans, and preparing families for ongoing support, including orientation to the Careforth platform. All assessments are completed within set timelines for prompt service activation. No night shifts or weekends required, and no on-call duties! What You Will Do
  • Manage initial contact with prospective families, educating them on service qualifications and assessing for service eligibility.
  • Conduct initial clinical assessments and evaluations, typically onsite in an assigned territory; guide families through efficient service activation.
  • Assign and orient caregivers to the Careforth app, Careforth's HIPAA-secure digital communication platform, supporting effective usage
  • Complete clinical assessment notes within a case management system, documenting medical history and any prescriptions; assist in developing a care plan and ensuring a smooth transition to start of service with ongoing case management.
  • Ensure audit readiness and compliance with regulatory standards.
  • Perform other related tasks as needed.

What You Will Bring
  • An active Registered Nurse (RN) License in the state of employment is required
  • Minimum of 2 years of recent experience in direct care of elders or adults with disabilities
  • Ability and flexibility to travel (usually by automobile) within assigned geographic area; valid driver's license and automobile insurance is required
  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
  • Tech savvy with the ability to teach others; proficient in common software and web-based applications (e.g. Case Management Systems, MS Office)
  • Ability to work in a fast-paced environment meeting department goals and KPIs
